This page is the full authorization you gave when you selected Yes on the War on Diabetes in the Black Church™ questionnaire. It explains, in plain language, what you agreed to and how to change your mind.
Your Authorization
“By selecting Yes, I authorize and sign for New Covenant Healthcare to contact me by call, text or email, and to use, license for payment, and share my health information, including information from my physicians, with healthcare providers, pharmacies, Medicare plans and their partners, for five years or until I cancel in writing.”
The sections below give the full details of that authorization.
Information Covered
- Your name, date of birth, gender, race or ethnicity, address, phone number and email address
- Your photo and the body mass index (BMI) estimate made from it
- Your type of health insurance (for example, Medicare or Medicaid)
- Your answers about health conditions, hospital or emergency visits, medications and health goals
- Your replies to our messages, such as confirming your height and weight
- Information from physicians who care for you, such as measured height, weight, BMI, vital signs and lab results
- If you ask for help with a prescription, whether a physician evaluated you and issued a prescription
Who May Receive It
- Licensed physician practices and telehealth providers, who may contact you about care that may be available to you
- Independent physicians and pharmacies, if you ask for help with a prescription
- Licensed Medicare plans and their agents and partners, who may contact you about plan options
- Service providers who help New Covenant Healthcare run this program
Why It Is Used
- To contact you about care, programs and plan options that may be available to you
- To keep your health information accurate and up to date
- To license Obesity Data™, New Covenant Healthcare’s consented health data
Payment
New Covenant Healthcare licenses health information, including yours, to the organizations listed above and receives payment for those licenses.
How We Contact You
- By call, text message (including automated texts) and email, at the number and address you gave us
- Weekly text messages from New Covenant Healthcare are not medical advice. Message and data rates may apply.
- Reply STOP to any text to stop messages, or HELP for help.
Important to Know
- Once your information is shared, the organization that receives it may share it again, and it may no longer be protected by federal privacy law (HIPAA).
- Saying No did not affect your ability to take part in the church event.
- Your church does not receive your health answers.
- New Covenant Healthcare does not provide medical care. Whether you receive care, and what care, is decided between you and a licensed physician.
How Long It Lasts
Your authorization lasts five years from the date you selected Yes, unless you cancel it sooner in writing at mrf@newcovenanthc.com. Canceling stops future use and sharing of your information. It cannot undo sharing that already happened.
